Information from this campaign continues to be shared and promoted by the local National Health Service.</p><p> </p><p>The National Stroke Service Model provides best practice for stroke care, including post-discharge, which should include comprehensive rehabilitation and personalised care and support.</p><p> </p><p>The National Stroke Quality Improvement in Rehabilitation programme is also helping to transform community-based care by increasing access to specialist stroke rehabilitation at home.</p>","originalAnswerText":"","comparableAnswerText":"","dateAnswerCorrected":null,"dateHoldingAnswer":null,"attachmentCount":0,"heading":"Strokes: Health Education","attachments":[],"groupedQuestionsDates":[]},"links":[{"rel":"self","href":"/Questions/1908284","method":"GET"}]}
